Requirements for your child taking medications in school
1. Written permission from your doctor with specific instructions
(i.e. dosage, time to be taken)
2. Your written permission for your child to take medications in school
3. Original bottle with the original prescription label. You can ask your pharmacist to give you 2 bottles.
School Physicals
Who needs them:
Pre-kindergarten Students
Kindergarten Students
Second Grade Students
New Students entering our district
Private Physicals
If you wish you can have your child's physical with your family doctor, and not have a school physical. You must have a copy sent to school by December, or a school physical will be done.
